Chaperone Information and Forms

A volunteer background check consent form must be filled out and returned to Mr. Rudolph before you chaperone unless you are a Cobb Co. school district employee.

You can sign up to chaperone on CHARMS.  For each game you sign up for you need to sign up for a practice within that month (2 games = 2 practices, 1 game = 1 practice, etc.).

Chaperone uniform for home games is chaperone shirt and jeans.

Chaperone shirts can be ordered using the forms in the envelope taped to the chaperone closet door (in the tuba room).

Acknowledgement of Volunteer Responsibilities under the Family Educational Rights and Privacy Act (FERPA)

FERPA Form

The Cobb County School district has mandated that any individual working with students will need to sign/agree to the FERPA form before they will be able to work with the students in the Cobb County district.  This form must be submitted EVERY YEAR for new and returning volunteers.

Individuals should proceed as follows:

1. Download the "FERPA Form" HERE,
2. Please open it in Word, fill in the editable fields on the 2nd page, save it and email it to [email protected]. If you would prefer to print it, sign it and send it as a scanned document or picture that is fine too.

VOLUNTEER BACKGROUND CHECK CONSENT FORM

Consent Form

The Cobb County School district has mandated that any individual working with students will need to have a background check before they will be able to work with the students in the Cobb County district.  This form must be submitted EVERY YEAR for new and returning volunteers.

Individuals should proceed as follows:

1. Download the "Consent Form" HERE,
2. Complete the information as requested. APPLICATION MUST BE TYPED. No handwritten background consent forms will be accepted.
3. Place in a sealed envelope with "Volunteer Consent Form" written on the outside,
4. Place in the white lockbox located inside the band room.
